A unique identifier in the url (/123-name-of-page or /123/name-of-page) could work, which is done by some sites. In practice, this means only the identifier is looked at (so the name-of-page can be changed without breaking existing urls). Urls like these are not suitable for all pages, though (e.g. a contact page should probably just be /contact, and urls that are used in printed media should also be short).
For articles, date-based urls could also be useful (to allow re-using titles/page names at a later point in time).
Regardless of the above, it seems a good idea to apply a title/name distinction to all pagetypes, not just articles.
This is getting more needed, since in De Stadsbron we now have articles like:
http://destadsbron.nl/nl/Addy_Schuurman_en_Eric_van_der_Velden
with the title: Bouwkoorts (4): 'Iedereen dacht: de sociale huursector kan wel krimpen'
This is because the first title was (by accident) the names of the authors.
And also rather clumsy:
http://destadsbron.nl/nl/bouwkoort_3 (missing "s")

Drupal has this mechanism as Matthijs hinted at. All pages are reachable through /node/[nid] where [nid] is the unique page identifier. Next to that you can change the url of the page, as long as there are no conclicts with same named pages.
